The patient assessments used in rehabilitation offer data to back up the creation of a rehabilitation plan that is specific to the needs, circumstances, and objectives of the client. To monitor patient progress, healthcare professionals must regularly record assessments. The patient is subjected to a number of evaluations, including the Motor Assessment Scale, the Functional Mobility Scale, and the Stroke Rehabilitation Assessment of Movement (STREAM). DocType's patient assessment tool makes it easy to record these evaluations at any point in the therapy plan.

1. How to Create a Patient Assessment
- Click New in the Patient Assessment list.
- Optionally, you can choose the Treatment Session that will take place after the evaluation. Information about the patient and the healthcare provider will be automatically obtained.
- A patient assessment template should be chosen. All of the assessment parameters and the description will be fetched automatically. The "Score" field in the Assessment table will be used to enter the patient evaluation scale.
- The evaluation can then be started by choosing a score for each parameter.
- Save.
- The individual parameter scores acquired will be added together to determine the Overall Score Obtained.
- In addition, you can add Time and Remarks while assessing each parameter in the Assessment table.
- Submit.
